Form Fill and Sign automates the entire life‑cycle of retirement‑plan paperwork—importing data, mapping it to PDF fields, and collecting compliant e‑signatures in one streamlined flow. Instead of chasing down PDFs and re‑typing data, TPAs upload a blank form, let Stax.ai populate every field in seconds, and send a secure signing link to clients and plan sponsors. Access the Form Fill & Sign Library

In the left navigation, select Libraries > Forms to open the Form Fill & Sign library of templates and questionnaires pre‑provisioned during onboarding.

Create a New Form Template

Click Add Form, enter a descriptive name (e.g., “Standard Company NDA”), and optionally add a description so teammates know when to use it.

Attach or Upload the Source Document

Select Attach File to choose an existing file, or Upload File to import a PDF from your computer. The document renders instantly in the editor.

Add & Configure Form Fields

Drag fields such as Text, Checkbox, Dropdown, Date, and Signature onto the page. Mark required fields and optionally Link them to plan, client, or project data so submissions write back automatically.

Save Your Template

When the layout looks good, click Save Changes. All fields are stored with the document so you can reuse it without re‑mapping.

Distribute the Form for e‑Signature

You have two options:

  • Share Link – copy a public link to send directly to signers. They’ll enter their name and email before completing the form.

  • Project Automation – in a project template, add a Request Form Fill & Sign task and assign it to a client or teammateSharing or automating form distribution

Track & Download Completed Forms

Open the form’s menu (⋮) and choose View Results to see every submission. Download individual PDFs or bulk‑export all results in one click.
